Going wider can help some high-instep feet, but width is not a perfect substitute for instep volume.
Measure both feet separately. A small difference is common; a large length, width or volume difference may need more than a standard pair.
A quality leather shoe can feel structured when new. It should not require pain, numbness or crushed toes as part of break-in.
