One Foot Bigger Than the Other — How Should You Size Dress Shoes?

Can a cobbler make one shoe smaller?
Not structurally. Pads and inserts can reduce usable volume, but they do not rebuild the last. Large asymmetry is better considered before the pair is made.
Should you measure feet in the morning or evening?
Feet can change volume through the day. Measure under conditions that resemble when you normally wear the shoes, and repeat if the result is close between sizes.

Length, width and instep can differ independently
The longer foot is not always the wider foot. One side can have a higher instep while the other has the longer toe length. Record each dimension separately.
Minor adjustments for the smaller foot
A tongue pad can reduce instep volume, a heel grip can fine-tune rear space, and a thin insert can reduce overall volume. Choose the adjustment that addresses the specific loose area.

Do not size to the smaller foot and expect the larger side to stretch dramatically. Do not stack multiple pads blindly until the foot is forced forward.
When separate sizing becomes relevant
Large asymmetry may justify individual review or different left/right specifications where a maker can support that process. That is a different level of service from normal made-to-order sizing.
How much difference is “significant”?
There is no universal threshold because a few millimeters of length, width or instep can matter differently depending on last and shoe type. The practical definition is whether one standard size/width can fit both feet without pain on one side or instability on the other.
Loafers expose asymmetry more than lace-ups
Laces can compensate for some volume difference. Loafers cannot, which is why a pair may feel perfect on one foot and loose on the other even when lace-ups in the same nominal size feel acceptable.
Build a left/right fit record
Write down which foot is longer, which is wider, which has the higher instep and which heel tends to slip. This prevents future fitting conversations from collapsing everything into “my left foot is bigger.”

Measure both feet, fit the more demanding foot, then solve small leftover volume differences on the smaller side only when necessary.
